What is Home Health Care?

 

Home health care provides services to people recovering from an illness, surgery, or rehabilitation who may need ongoing assistance and monitoring in the comfort of their own home. Under Medicare rules, a person qualifies for in-home care if he or she is homebound and needs skilled medical care.

Who Pays for Home Health Care?

 

Most health insurance companies, HMOs, PPOs and Workers Compensation cover home health care. In addition, Medicare and Medicaid pay for home care services. Some insurance providers do not cover all home health services. Our staff will verify health coverage for the patient.

Do I have to be hospitalized to have home health?

 

An individual is not required to have been hospitalized or discharged from a skilled nursing facility before they can receive home health care. Most patients are referred directly from their doctors office.

Can I just receive rehabilitative services at home?

 

Yes, a nurse, physical or occupational therapist can be sent to a patients home to evaluate for home safety and recommend assistive devices for a patient. Generally, the therapist, along with the physician, will develop a home exercise program to fit the patient's needs.

What services does the nurse provide?

 

Many medical conditions that previously required hospitalization can safely be treated in the home. Home care services may include but are not limited to:

 

  • Observation and assessment

  • Patient and family education of disease process

  • Management and evaluation of patient care plan

Yes we do. Our dedicated and skilled clinicians help coordinate the care your doctor prescribes. In support of your doctor's orders, we develop a written plan for your care. It explains what services you will receive in order to reach your best physical, mental, and social well-being. At Healthcare Plus, we pride ourselves on staying in touch with your doctor. In fact, your doctor will be able to access your visit notes 24/7 through our secure, online, HIPAA-compliant Physician Portal.

How do I schedule my first visit?

 

Talk to your doctor or call Healthcare Plus at 847-776-0800. Once we receive a physician's order for care, we will set a date and time for your first visit from a Registered Nurse. We will do our best to accommodate any schedule request you may have.

What should I do in case of an emergency?

 

In case of an emergency, have someone contact your doctor, or if necessary call 9-1-1.

What should I do if I am admitted to the hospital?

 

If you are admitted to the hospital, please have someone contact the Healthcare Plus at 847-776-0800 to inform us of your status. When you return home, please call our office again to restart services and continue care.
